The highest-earning role in nursing requires a specific path: BSN, ICU time, doctoral CRNA program, and NBCRNA certification. Here is exactly what that looks like, how long it takes, and what you earn when you get there.
Certified Registered Nurse Anesthetists are the highest-paid advanced practice nurses in the country and one of the highest-paid healthcare professionals overall. CRNAs administer anesthesia for surgeries, labor and delivery, pain management procedures, and emergency interventions. In 24 states and the District of Columbia, CRNAs practice fully independently without physician supervision. In every state, they are the primary anesthesia provider in most rural hospitals.
The path is long and demands serious ICU bedside time before you even apply to a program. There are no shortcuts. But if you come out of a good ICU with your CCRN, a competitive GPA, and a genuine grasp of hemodynamics and pharmacology, the doors are open.
I spent years in the ICU watching CRNAs work. The ones who got there were not the smartest people in the room — they were the most methodical. They could explain exactly what a drug was doing to a sick patient’s physiology. If that is how your brain works at the bedside, CRNA is worth every year of the journey.
Every CRNA path starts with an RN license, and CRNA programs require a BSN (or higher). Associate degree nurses must complete an RN-to-BSN bridge before applying. Most traditional BSN programs run four years; accelerated second-degree BSN programs take 15 to 18 months. Online RN-to-BSN programs typically take one to two years for working nurses.
Your BSN GPA matters. Most competitive CRNA programs want to see a 3.5 or higher, particularly in science coursework (anatomy, physiology, chemistry, statistics). If your undergraduate record is uneven, a post-baccalaureate science course retake or strong CCRN performance can partially offset it, but a sub-3.0 GPA is a hard barrier at most programs.
Science prerequisites most programs require: Anatomy & Physiology I and II, Chemistry, Organic Chemistry or Biochemistry, Statistics, and Microbiology. Get these done before you apply to CRNA school, not during it.
After graduating from an accredited nursing program, you must pass the Next Generation NCLEX-RN (NGN) before practicing. The exam was updated in 2023 to include a Clinical Judgment Measurement Model with new item types: Case Studies, bow-tie questions, matrix grids, and trend interpretation. The first-time pass rate for US-educated candidates remains above 80%.
Most CRNA-bound nurses do not stop at the NCLEX. They also pursue the CCRN (Critical Care Registered Nurse) certification from AACN after they gain eligible ICU hours. The CCRN is not always required for CRNA admission, but it signals genuine critical care competency and appears in the application materials of most accepted candidates at competitive programs.
Every COA-accredited CRNA program requires a minimum of one year of acute care nursing experience, and virtually all competitive programs expect two or more years. More important than duration is the type of unit. Programs prioritize:
Step-down, PCU, PACU, or procedural sedation experience alone is generally not sufficient. If you are currently on a step-down unit and want to move toward CRNA, the first step is to lateral transfer into a level-one ICU as soon as you are eligible.
During your ICU years, build your knowledge base deliberately. Read Clinical Pharmacology Made Ridiculously Simple. Understand your vasopressors cold: mechanism, dosing range, receptor profile, hemodynamic effect. Attend code reviews. If your hospital offers shadowing with the anesthesia team, take every hour they offer you.
The ICU years are not just a box to check — they are where you develop the physiologic intuition CRNA school expects you to have on day one. Programs are not going to teach you what dopamine does to a septic patient’s hemodynamics. You are expected to know that from your bedside time. Use every shift.
As of 2025, all nurse anesthesia educational programs must be at the doctoral level to maintain accreditation through the Council on Accreditation of Nurse Anesthesia Educational Programs (COA). The master’s-level CRNA pathway no longer exists for new students. You will graduate with either a:
There are approximately 120 COA-accredited programs in the United States. Programs vary in length from 28 to 36 months, with most running three years. All include advanced coursework in pharmacology, physiology, pathophysiology, and anesthesia principles, combined with at least 600 to 800 clinical hours across a range of surgical settings.

The first year of a CRNA doctoral program is heavily didactic: advanced pharmacology, anesthesia principles, regional techniques, airway management, and simulation lab. By year two, students are in clinical rotations across general surgery, obstetrics, pediatrics, cardiothoracic, neurosurgery, and ambulatory care settings. Year three typically includes capstone research or a Doctor of Nursing Practice project alongside continued clinical hours.
Programs that integrate rotations in rural hospitals or Federally Qualified Health Centers prepare students for the reality that CRNAs are the sole anesthesia provider in a large fraction of US hospitals. Over 65% of rural hospitals depend on CRNAs as their primary or only anesthesia professional. That is where the job market is deepest and the autonomy is highest.
You will not work during CRNA school in any meaningful clinical capacity. Budget accordingly. Full-time tuition runs $30,000 to $80,000 per year at most programs. Federal loans, institutional scholarships, and the Air Force, Army, and Navy nurse anesthesia programs (which pay full tuition in exchange for active-duty service commitments) are all valid funding paths.
After graduating from a COA-accredited program, you are eligible to sit for the National Certification Examination (NCE) administered by the National Board of Certification and Recertification for Nurse Anesthetists (NBCRNA). The NCE is a computer-adaptive exam covering anesthesia practice, principles, and pharmacology. Most candidates sit within 60 to 90 days of graduation.
The NBCRNA recommends taking the Self-Evaluation Examination (SEE) as a practice assessment before the NCE. Most CRNA programs integrate SEE prep into the curriculum. The NCE pass rate for first-time candidates has historically exceeded 85%.
Recertification (since August 2025): The NBCRNA replaced its Continued Professional Certification (CPC) program with the new Maintaining Anesthesia Certification (MAC) Program. The MAC runs on a four-year cycle with three components: MAC Check (quarterly longitudinal knowledge assessments), MAC Ed (continuing education credits), and MAC Dev (professional development activities). CRNAs who completed CPC cycles before August 2025 transitioned automatically.
CRNAs and anesthesiologists perform the same clinical tasks: pre-anesthesia evaluation, induction, maintenance, emergence, and post-anesthesia care. The educational paths differ: anesthesiologists complete medical school (4 years) plus residency (4 years), for a total of 12 or more years of training versus 7 to 10 for a CRNA.
In practice, the role overlap is substantial. In non-supervision states, a CRNA manages complex cases including cardiac, obstetric, pediatric, and neuro surgery cases entirely independently. In supervision states, a supervising physician is nominally present but often managing multiple rooms simultaneously. The CRNA is doing the hands-on anesthesia work in both environments.
The pay gap has narrowed considerably as more states adopt opt-out supervision policies and CRNAs practice in higher-acuity settings. The mean CRNA salary of $248,320 sits in the same range as hospitalist physicians, emergency physicians in lower-cost markets, and primary care physicians in most of the country.
